very slow computer- can't identify the cause [RESOLVED]

I was given a friend's computer to clean up and fix after he started having severe slow-down problems with it. Every program takes forever to open or change, the boot process is unbearably long, and Windows Installer didn't work until i manually reinstalled it (didn't speed up comp though). According to task manager and process explorer (sysinternals), the cpu and memory usage never go above 50%. Granted, the computer only has 768mb ram, but it used to run fine. I donno what my friend was doing when the change happened, but i tried system-restoring to a long-ago date, but to no avail. Computer runs perfect fine in safe mode, but lags terribly in regular. I even created a new user to test whether the lag was user-specific, but it still persisted. The usual spyware tools did not reveal any new viruses or threats, and a look through the hijackthis log only reveals the BHO items, which should not affect me as i use firefox. The log is attached, and I would appreciate any help at where to dig to fix this problem. Thank you.
