Problem exporting OE6 address book to .csv

When I try to export my Outlook Express 6 address book to a .csv file, most of the names are missing. The email addresses all show up in proper order, but three quarters of the name boxes are blank in the resulting Excel file. I get no error messages, and the names are properly entered and appear normally in the address book window in OE.
Earthlink Webmail requires a .csv format to accept the address book. Any help greatly appreciated. This seems to stump everyone, and I can find nothing on it in the forums I have searched.

Thanks, JWD
