Samsung has a new warranty that guarantees no dead pixels on some monitors, but most manufacturers require 4-8 or more pixels before they replace a monitor. In fact as many as half LCD panels ever produced never see the light of day because they have too many manufacturing defects and are scrapped. An average LCD has more than a million pixels, if just one of them are defective, then it'll result in a dead or stuck pixel.
Depending on the position and color of the pixel, most manufacturers will make exceptions to their rules. For instance a stuck red pixel is usually considered the worst, and if it's center of the screen, most mfgs will replace.
