HTML Web form to be saved as a doc

I have created myself a web form that is being used on our intranet.

What I need is to be able the offer of saving the form in a word doc format once all the values have been entered (check boxes, radio boxes, drop down lists, text enter boxes).

If I try and do a Save As... I keep loosing the filled in data and does not format correctly in word.

Any ideas?
