looking to put a poll on my website

Hi guys,

So I'm actually creating a malware website at the moment and i'm looking to find out how to create a basic poll that I could put on the homepage.

One with radio buttons with 4-5 selections then once submit is click it brings up the results in a graphical (bar) format and also displays the different percents. My HTML knowledge is pretty limited so I was wondering if someone could walk me through it or point me some where with a good guide.
