Knee deep in infections! Help wanted

First time user here, so bare with me please.
I came to check on my little brother's PC to find all sorts of malicious software intrenched on it... have spent HOURS and HOURS cleaning as best I knew.. AdAware, Spybot S&D, AVG, all updated.. Finally saw forums like this, and dl'd Hijack This with hopes of finding someone who can get me on the road to cleaning this machine, as alone I keep running in circles.

I have tried deleting the isrvs folder, files within the folder, yadda yadda, even had problems removing registries with regedit.... i'm exhausted and need help!
