Design to psd?

Do you (always) make your design as a psd or similar file first? Do you make a sketch on paper? How often?

I forgot to say what i do lol:
Never used graphics program for this.
I did a piece of paper maybe twice.

I use Photoshop 10% of the time...
A piece of paper 50% of the time...
My idea worked out on the spot in Dreamweaver 40% of the time...
