If you want real time I think SpywareGaurd offfers RealTime for free. If you want something to run on your own time. Ad-Aware SE and SpyBot are very good.

If you are also in need of a good firewall too, I use 'Agnitum Outpost Pro' which includes a real-time spyware detection plug-in with auto-update. So you can 'kill two birds with one stone' instead of having separate applications for firewall and spyware.
