Virus scan problem

I can't seem to keep mcafee's on-access service running on a machine. It had a LOT of problems (virii/spyware/etc) all of which seem to be taken care of except this. Can someone give me an idea of how to get this up and running? Without it, i'm sure the machine is in for the same trouble it had before...

So is this a virus causing this? Any idea which one? Any idea how to start virus scan before windows fully loads and catch this virus?
