Windows appears to shut down...computer won't turn off

Don't know what brought this on 2 weeks ago. I scan for everything religiously. No apparent bugs, spies, etc. Windows shuts down, computer won't. I've checked out forums, and http://aumha.org/win5/a/shtdwnxp.php, which I've had no luck. I've done clean boot.

Need help...also, OE spam suggestion. I have a loaded "blocked senders list". Can this bog down Outlook Express?
