Display loss during game

During a game (3d application) my monitor goes black and "Analog" and "Digital" are flashed in a box in the upper left corner. I recently bought a new Samsung LCD monitor and it worked fine on it for a while (it also worked fine on my old Sony CRT). This problem only started occuring recently.

Is this a hardware or software issue? Monitor or vid card?

Any help would be appreciated.
