Since mid-January I've been having problems with CS:S, and when I picked up COD2 last month I ran into the same problem. I've tried Radeon X1650Pro, X1600Pro and X850 Platinum Edition and the same problem exists. I've reformatted my system twice, tried numerous installs of DX9 as well as the drivers most of my hardware. Today I tried Catalyst versions 6.4 and 6.6 after reading a post on another site to no avail. If anyone's encountered this problem, let me know. I'm pretty sure this is a software issue, despite what ATI's helpline told me when I called when I first started having the problem with the X1650. Thanks for reading!
Here's my log from COD2:

if you have reformated your system and have loaded all the latest drivers and updates from microsoft then you should not be having any problems.

Please go to start, run and type in dxdiag. On the very first screen, last line is the version of direct x you are running. Under each tab is a "notes" section. Go through there and see if any problems are found.

I would make sure you have the latest version of chipset drivers for your system. Specifically the gart driver.

DX version: 9.0C (4.09.0000.0904)
I ran all the tests in dxdiag, everything worked fine. No problems were present in notes.
All of my drivers are up to date, including my Nforce4 drivers. Everything SHOULD be working, but for some small overlooked detail somewhere, it's not. I'm not the only one having this problem (Google "Direct3DDevice9::Present failed: Driver internal error", and you'll see what I mean) but no one seems to know the reason. I checked the Steam forums for similar problems and all I find is an alert that went out saying that it's an as-of-yet unsolved bug. A buddy of mine suggested that it might just be a problem between my Nvidia chipset and my ATI Video card, but I couldn't see either manufacturer overlooking such a combination. I appreciate how fast you responded Starjax, now I just hope there's a simple solution out there somewhere...
