So I screwed up. I got some sort of malware

I was downloading a program a while ago and something bad hitched a ride along with it.
My computer has had malware before and I've removed it but it's never ever been this difficult to get rid of.

Some symptoms is the desktop items competley vanishing leaving only the background picture. I use Control Alt Delete and pull up the task manager to bring things to the front. It also has annoying pop ups saying my computer is infected with Worms, Adware and Trojans among other things.
It also opens random internet pages.

I've ran McAfee, Spyware Doctor and VundoFix. All of which have failed to fix the problem.
When I ran the spyware doctor the first time it found 157 infections.

Just for the record my computer also has technical problems which have been going on for some time and I believe are totally unrelated. It just shuts off but the on button remains lit up.

McAfee comes up sometimes and says that it has found a potentially unwanted program called Winfixer. I select the remove option but it doesn't fix the problem.
