PC Doctor Hard Drive Error, slow system, etc. PLEASE HELP!

I ran the PC Doctor and on the hard drive test, it said fail in the SMART short self-test part and gave error message HD521-2W

My system has been having a memory read error when trying o reboot.

The system is also verrrrrrrrrrrryyyyyyy slow and at times receives a BLUE screen where it states my memory is being dumped unless I reboot immediately.

I am beyond a confused girl with this - I used to work on computers (long time ago) but am at a complete loss with this.....

CAN SOMEONE PLEASE HELP ME???
