Hi,
I'm frustrated and behind schedule for work and am hoping someone can help me with placing form fields.

I have a form that is already created in word 07 (compatible with 97-00). It basically has text with lines that follow (made of underscores).

For example:

Name:_______________________________________

Date of Birth:_________________________________

Place of Birth:________________________________


I need to place a form field on the line so that when the text is entered, it is centered on the line.

For example:

Name:________________John____________________

I NEED IT TO LOOK LIKE THIS (AFTER USING FORM FIELDS)

Name:________________John______________

Currently, when the fields are typed into, the lines around it advances, leaving plain text in the middle.

Once this is formatted correctly, I also need to make sure that users cannot edit or change the original format of the document.
