Please help, smith fraud wont go away

My mother has the smith virus or whatever it is called. We ran the smithfraud fix and it did not work. we also did another removal tool that said it would fix it and it did not either. it is not showing up in add remove program list.
We ran Hijjack this, here is the log.
Please note we know very little of how things work, so be very simple and clear.

Hello killertatertots and welcome at Geekstogo,

I am afraid I have bad news for you.

One or more of the identified infections is a backdoor trojan.

This allows hackers to remotely control your computer, steal critical system information and download and execute files.

I would counsel you to disconnect this PC from the Internet immediately. If you do any banking or other financial transactions on the PC or if it should contain any other sensitive information, please get to a known clean computer and change all passwords where applicable, and it would be wise to contact those same financial institutions to apprise them of your situation.

Though the trojan has been identified and can be killed, because of it's backdoor functionality, your PC is very likely compromised and there is no way to be sure your computer can ever again be trusted. Many experts in the security community believe that once infected with this type of trojan, the best course of action would be a reformat and reinstall of the OS. We can still clean this machine but I can't guarantee that it will be 100% secure afterwards. Let me know what you decide to do. :)

Well we cannot afford to do anything that involves spending money right now. We definitly want to try to clean it now, and then look at other options once she is working again. She doesnt do any banking online, but should she change her passwords for everything else too?

Update- we have tried using all the programs we could find that say they can fix it, they dont work. We need help. We cannot take it in to get fixed, please, you said you could tell us how to fix it, please help, it has been days, she needs her computer for work, we cannot wait forever, no one can help us, nothing is working.
