Remote Desktop - router problems

Hello, let me explain.

I work in a place. lets call it WORK. And i have a PC connected to a local netowrk.

If i want to connect from home, i need to dial a VPN connection and after that i just use Wondows Remote Desktop. No problem here, but is before getting a router at home.

Now i am able to connect the VPN (or at least it connects and gives me internal ip) but i cannot connect to Remote desktop.

Example: my external IP is 89.200.200.200 and my internal ip givven by the router is 192.168.100.100.

when i connect the VPN i get another ip 16.10.10.10. What do i have to do to the router so i can connect to my work pc?

All i can find on the internet is how to connect from a remote location to a pc behind a router, but this does not help.

I hope i explained it ok.
