Hi!

I thought I was pretty good at this- and well protected- guess not this time! Help!! I assume this is malware or spyware of some sort

The pesky startup command that my friend Scotty (WinPatrol) keeps popping up and telling me that a rundll32.exe wants to run at startup. The command line has changed its' dll's file name three times (name appears to be randomly generated and changes every hour or so after I keep rejecting the startup?)
is this one:
O4 - HKLM\..\Run: [Adopidurayapev] rundll32.exe "C:\WINDOWS\ociwuzozawufile.dll",e
I assume something else is calling it, but I can't tell which process??

Can't delete this through msconfig or any other process- it just pops back up. Grrrrr...

See entire hijackthis file below:

When I attempted to run your comedian.exe file, AVG immediately detected Trojan Horse Backdoor.SmallX.Vx What is this?

Regarding your other suggestions, I downloaded that Malwarebytes software and it picked up seven bad entries. It seems like the MSAntiVirus2009 bug- not sure how I contracted it.
