Well, wouldn't you know it? There is not MP4 listed. I hate causing you all of these headaches, but I am going step by step like you say and this step is not listed. :)

It's hard to believe, but, you can use any media file extension, so instead of .mp4, you can try .avi, .mp3, .cda, .mov...
Some of them have to be there.
