Multiple system crashes

Hi,

My system has been crashing about once a day for the last week or so. Today it already crashed 3 times. By crash i mean the screen just freezes, turns partially black and shows a BSOD a few seconds later.
Below is the report from bluescreen viewer for today's crashes.

I've ran chkdsk and memtest86+, but no help. The pc was recently formatted due to similar problems. ( I would get a disk read error and couldnt even boot into Windows, this problem has disappeared since the format)

Any ideas as to what could be causing this?

Probably your video card driver needs to be reinstalled, or your video card is going bad.

Try to reinstall video driver, first.

Is it on-board video, or separate video card? Desktop, or laptop?

It's an ati HD4850. I updated the driver from 8.11 to 8.12.
I'll report back if/when it crashes again.
