Video card acting up

Hi, I'm using a Geforce 6800XT on XP and my 3D models are all messed up in game and in the NVIDIA control panel. I don't even know where to begin to try and fix this but I've already tried reinstalling my drivers and DirectX.

I believe it is linked to DirectX because when i run a game(counter-strike:CZ) in openGL everything works fine.

is this fixable or is my video card broken?
Piriform Speccy says that the temperature of the card is 74
