Do you have to download virus protection, and malware protection programs onto an external hard drive before you store any files on it?

you can don't have to.
what you can do if you don't trust a file is scan the external with the virus scan that is installed on the desktop/laptop your are using.
In My Computer . . right click the external drive and select Scan with { name of AV }
