I've got:

Toshiba Satellite L455D-S5976
Windows 7 -32 bit
AMB Sempron Processor SI-42
2 GB DDR2 RAM, 250 GB HDD
Radeon Graphics card

This is my brother's college computer. He said it crashed about a week ago and sent it to me.

Normal start up will not work. I can get it to run in SAFE MODE, but it is very slow. When it finally makes it to the desktop, Windows Help and support pops up, but within about a minute WINDOWS EXPLORER restarts.

I've run system repair. It has ran two times. This repair action failed:

Repair Action: System file integrity Check and repair
Failed. Error Code: 0x45d

I have ran the CCleaner and repaired items on the registry. What needs to be done to get this back up and running?

have you tried Last Known Good Configuration?

You get there the same way you got to safe mode, just chose "Last Known Good Configuration" instead.

I have and it doesn't do anything at all. When I tried to do it again, it says there isn't one. :S

I only have 2 things that I can think to tell you. A third option would be to wait for someone else to chime in.

option 1 if nothing is of value on it I would restore / reformat the HDD

if there is valuable stuff on it you might want to go to the malware removal forum and have them take a look at it.
