Weird Icon on my screen, overlaying everything else

a few days ago, while surfing, this weird icon (Win 3.11/95 style) popped up and stuck there until reboot. It was overlaying everything else, and I could not move it, close it or put in the background, it just stuck there. I fired up ProcessExplorer but could not find anything out of the ordinary. After a reboot, it was gone. Just for good safety, I reset the system with a previous system image.

That said, I would really like to know if someone has an idea what this could be - if this is malware related, it could still lurk somewhere deep in my system, as I have no idea when I obtained it (if it is malicous, that is) and whether it is also on my previous system snapshots.
