Also (I just noticed, after running OTL the other day) all my video files now show their extension for some reason (.mp4 etc) and I can see all sorts of folders/drives that are normally hidden.  For example, my 'My Computer' shows a bunch of virtual drives that daemon tools used, as though they're there, albeit greyed out.  Any idea why?

Sorry for the delay in getting back to you in the malware forum, we are quite busy lately as you can see. Please be patient, if anybody won't pick up your thread in the Malware Removal, I should be able to review it later this afternoon, hopefully we will be able to find a solution, or at least point you in the right direction. 

Do not worry about the extensions and other stuff after running the OTL scan. Malware removal has its own specification, we need to see as much details as we can. After completing the cleaning your settings will come back to their standard options.
