Formmail attachment problems

Hi all

Wonder if anyone can help me please?

I have a web page at http://www.perfect-proofreading.com which calls on formmail.php. When I upload an attachment, it doesn't make it through to me. Can anyone take a look at my form and help please? I've copied the php file below.
