Suspected dead GPU, not 100% sure. Looking for advice!

So, while starting up a game the other day, my computer locked up (no video, no keyboard response, audio still running). After restarting, I would get past the Gigabyte BIOS logo screen, then the machine would freeze and a white horizontal line appeared in the upper half of the screen, with a series of colored dots just above it. I was able to enter BIOS after restarting again, didn't seem to see any missing hardware or other issues (it doesn't report GPU stats). I would get the exact same artifact pattern on the screen each reboot, followed by total lock-up.

I assumed a dead video card, so I removed it and booted up with my monitor connected to the mobo via HDMI, and I booted into Windows normally (not safemode) with onboard video. It's been going for a day with no issues (other than not having a proper GPU!).

My issue is this: I'm concerned that maybe the PCIE slot on the mobo went bad and the card itself is actually still good, but don't have another card to test in it. Also, I experienced some random restarts in past weeks, but they seemingly only happened in a specific game (Deus Ex: Mankind Divided). So, now I'm thinking maybe my PSU is faulty or maybe even under-strength for the card (though everyone I asked said 650w is enough for my setup).

I guess I'm curious what you guys think; just a case of faulty GPU, or could its death have been caused by secondary issues like the mobo or PSU?

Other notes:
- Power lights on the card are still on and fans are running
- Heat has never been an issue for any of my components
- Nothing I have is currently overclocked
- No recent driver updates to the GPU

- Win 10 Home (64-bit)
- Intel i5-6600k 3.5ghz
- 16gb RAM (g.skill Ripjaws V Series)
- Gigabyte GA-Z170XP-SLI (rev 1.0) mobo
- Samsung 850 EVO SSD (500gb)
- WD Blue 1TB HDD
- Corsair RMx Series 650w PSU
- Geforce GTX 1070 (ZOTAC Amp)

Suspects, 

PSU, possible low 12V rail.

Faulty GPU.

Faulty Slot. 

 

Test slot,   Start by swapping the GPU to  the second PCI Express x16 slot, running at x8 (PCIEX8)

 

Test PSU.   Preferable with the GPU running.  Download/run HWMonitor, take a screenshot, save and upload.  > http://www.cpuid.com.../hwmonitor.html
