How do I remove TSS VinCE?

Our program Malwarebytes Anti-Malware can detect and remove this potentially unwanted application, but due to the nature of the infection this will require booting into Safe mode with networking.

Note that there are several variants of this one out there. Some are using different filenames and phone numbers, but the blue screen is the same (apart from the phone number) and they can all be removed using this method.
