Unidentified Trojan

Hi guys. I have had what I believe is a trojan for the past 3 weeks. The internet seems to run in cycles now, meaning it will be fine, then bog down and slow to a halt then 3 minutes later be fine then bog down , etc. It's a neverending cycle. I've gotten almost every program out there and they've erased things on my computer but I still have this problem, and it's really getting irritating. Here is my HIJACK this log:

The only part to me that looked suspicious is this line:

Someone mentioned it could be my isp but i never seen this before in my logs in the past. Any help would be greatly appreciated. I'm on my last nerve here.

A friend of mine said that also if nothing is showing up, my net card could be on its last legs. Before i dwell into hardware issues though i want to make sure the software is ok

New netcard didn't work so i'm out of ideas. Can someone please take a look at the log and tell me if anything is out of the ordinary?
