I need help! Im not really good at this whole thing. But i want to make like a thingy that shows questions and the person can answer then(not like a quiz). and then i be able to see what the person answered for the questions.

Like a form that has questions and you fill out the questions and when u hit send it send the questions and answer that the person did to an email, or to a database. but i dont wnat there to be like a set answer for the questions. the answer to the questions is like about the person. not a quiz or a poll or anything like that. like a questionaire or something like that.
