adding an instance

I have been given a set of sample code in a powerpoint presentation and i am trying to fit it all together just to see how it works. i need to add an instance to the program but i cant work out where it goes.

i have attached the power point presentation as it wouldnt let me attatch the actual code. the instance is displayed on slide 15. if it needs to be put in a new class what would i have to call it? if it just needs adding to an original class where abouts does it need to go?

Any help would be appreciated.
