Go to Start->All Programs->Accessories->Communications->New Connection Wizard
Or if you can't find that, go to Start->Run
Type rundll32.exe netshell.dll,StartNCW
On the wizard welcome screen, click 'next'
Default option should be the top one which is the one you want, so click 'next'
Choose option "set up my connection manually" and click 'next.
Choose default option "Connect using a dial-up modem. click 'next'
Type the name of the ISP who supplied the dial-up number (this is not critical, it's just a name for the connection).
Follow the wizard through from here, supplying account name, password, and dial-up phone number when prompted (these are critical, must be correct).
When done, you'll have a dial-up shortcut on the desktop.
Open that shortcut and click 'Connect'.
