Help Uninstall Second Vista Operating System

I did a reinstall and now I have two Vista operating systems on one partition, one on C drive that I want to keep, and one on D drive that I want to remove. I have tried to format the D drive in Vista but get a message that Vista is "unable" to format it.

How do I remove the unwanted installation?

Have you tried using a boot disk such as BartPE and formatted from there?, and have you removed the second vista from your boot.ini?, if you haven't, do those now!
