New Build – ATI Graphics driver stops responding when Gaming

I was hoping someone could help. I have recently built a new machine. I have used my graphics card from my old machine, an ATI HD6950.

Issue:

Over the last couple of weeks, every time I have been playing GTA V or any other game like The Elder Scrolls loads of small squares appear on screen and then the game crashes. I see the error “ Your graphics card driver has stopped responding and recovered successfully” This still crashes the game though.

I believe I had this with my older machine before the upgrade but to be honest cant be 100% on that. The most likely thing to me is the graphics card. I want to be sure before I purchase a new one.

My Attemps at resolving:

Updated Drivers – Graphics motherboard etc
Updated BIOS to latest
MS Updates
I even did a clean re-install of OS just in case.

So my question.  As anyone had this error before or know how to fix it? I have seen a lot of conflicting advice but none have pointed at the graphics card. Some ideas that the CPU needs to be Overclocked slightly as it may be unstable?

Thanks

You hardware is good, PSU strong, CPU fast enough, don't think you need to O/C it.

Check to make sure the GPU has not been inadvertently O/Cd.

Monitor both temps/voltage for both CPU/GPU and post some screenshots.
