dial up modem trouble logging on to Internet

Did a registry "unclutter" with Glary couple of weeks ago, might have messed something up. Tried to use a registry backup file, tried system recovery, to no avail.
If I go to a date farther back in restore mode, something tells me that may not "fix" my connection problem.
Got in touch with isp, for some reason they had a totally different user name/password for my logon, which has supposedly been changed to what I had. They supposedly had a switch in their systems used.

Getting a 691 error code.
Looking at my modem logs it looks like there are errors-port not configured by matching in system ports, port not configured frim nt4 conversion; StringCchCopy failed due to error 0x8007007a
Trying to match phonebook port in the system ports.

This is a USRobotics 5639-00R usb modem, I have Win 8.1 AMD64.

I did a full boot scan, did not show anything.

There is a pdf note about Customizing the AT10 response string-never did that, modem worked fine. Via Control panel, modem query says modem works.

I uninstalled the modem via windows uninstall, reinstalled it. I made an exception to allow the modem through my firewall and an exception through Avast.

Any help would be appreciated!
