Here's what I need to create and don't know what program or app to do it in...

1) Create an email "fill-able-form"  questionnaire that asks what people want for Christmas, their hobbies, etc.

2) I email this form to (40) family members to each fill out and save the file as their name.

3) The form uploads to one place where the entire family can access and click on whoever name they want to see.
