Problem with the submit button on a form

I'm kind of new to editing HTML and I need help making a form and I'm stuck making the Submit button. I'm sending it to a file called html_form_action.asp and whenever I click it, it just says there's an error. I want it to lead to a page where it shows the form they filled out with their answers and an HTML code for them to post their answers to their profiles and stuff.

Your form tag should have the path where the form is to be processed when submit button is pressed within the action="" attribute.
