Win2Kpro - File Type Icons in Windows Explorer Incorrect

Is there a reason for this, and if so - can it be fixed? I have done all the usual 'icon' fixes related to the icon cache, however this anomaly does NOT affect my desktop, quick launch, or shortcut icons. It only shows up within Windows Explorer and on the Start Menu. Double-clicking on opens whatever file in its correct application, it just doesn't show the correct icon - very frustrating, especially the representation for drive icons. Screen cap samples attached. Thanks for any input received.
