extremely slow on start up screen, sound jittery

Hey guys my computer is running extremely slow. I just formatted it and reinstalled windows xp and reinstalled all the drivers. it was working fine for a week or two and it went back to the same problem. The start up screen stays on for about 5 minutes compared to the usual less than a minute, and when the windows sound plays when it starts up it is extremely jittery and you can tell the computer is working too hard. I have no idea whats wrong.
I have SAV 10 installed and no viruses. Someone please help, i could just reformat again but the problem comes back in a week. Do you think it could be my hard drive is shot?
