Trojan keeps changing internet connection settings

Hello,
there is a real nasty SOB on my machine. I'm fully patched up, got up to date AV and spyware, hardware firewall, and just can't find this thing.

When I go to connect to the Internet (via dial-up) the trojan keeps altering the phone number in the internet connection settings as well as opening up pages on lots of sites. It likes to hide, I think I've fixed it and its fine for 20 minutes, it then it kicks in again.
