Graphics card dead?

Hello!
I got a graphics card about 1 1/2 years ago, ATI Radeon x700, and it work(ed) great! I just build a computer 1 month ago and thats were the problems start. My graphics card was running great til' about 2 weeks ago. I started to get a screen with transparent yellow rectangles that make a checkered pattern across my screen. I decided to re-install my graphics card's driver and it worked great til' now. The checkered thing came back. Is my graphics card on its last leg?
