iPhone 6s Plus being spied on?

I'm not sure exactly what is going on, but I feel like I'm going crazy. Searches I never did are showing up in history, or even when I open browser, it's a page I never went to. Things are changing all over my phone. Notes, documents, pictures, settings.

Did you really need to make three topics?

 

Surely if you're being spied on then due to the nature of spying such searches and settings would not be changing. I doubt you're being spied specifically, there may be something else however.

Best possible way would be take take backup of documents, photos, videos, logs, files etc and factory reset the iphone.

Did you have your iPhone 6S plus connected to a computer like a Mac? So far I know, Apple IDs would sync almost anything like pictures, Safari searches, musics, etc.
It's almost the same like Google accounts.
