Mal/Spy ware

I was wondering if some one here would be so kind as to help me.

I am having alot of troubles removing some Mal/Spyware from my machine.

Microsoft Antispy, Ad-aware and McAfee are picking up nothing and i do not see any unfamiliar entries into my systems startup.

Anyhelp would be MORE than appreciated. Below is a screen shot of the bugger.
