Malware Infection - Please Help

Here I am again - Malware still here (winfix pop ups)
The only thing that I see that is the same as the last time I was infected is....
I get a rectangle shaped gray box saying a browser homepage change was in play - do you want to
stop the change or allow it. When I clicked the button, it seemed like things started from there. The only problem is.... the warning box said "SpywareGuard" - which lead me to believe that it was safe to click the button to stop the browser homepage change. Is there some malware out there posing as "SpywareGuard" ?

I have gone through "every" step in the "do these things first - so you're SUPER protected" list.

Where would you like for me to begin?
