Modem won't work in Windows 98

I'm trying to get my modem working using windows 98.

The modem worked fine but then something happens and windows wouldn't
boot up anymore. I Formated my harddrive & reinstalled Windows 98.

Now the modem won't work. I have tried to add a modem using the Modem
Driver Disk that I have but when It gets to the point of telling it what
Com Port to use, Nothing is available to pick from.

This is a friend machine, so I really have no idea about what type of modem
it is.
